COVID-19 Vaccine Scheduling Information

Depending on state allocation, we are beginning to vaccinate patients at several of our health centers. If you have been notified that vaccine is available at your QuadMed health center and you have received a notice to schedule your vaccine online, please log in to or create a MyChart account, select schedule an appointment and select COVID-19 Vaccine as your appointment type.

Important Patient Notice

Due to COVID-19, not all visit types can be scheduled online at this time. You may be directed to call the health center to schedule your appointment. For safety and convenience, virtual visits are also available. Learn more about virtual visits at myquadmed.com/virtual.

21st Century Cures Act

In compliance with the federal 21st Century Cures Act, beginning April 5, 2021, additional clinical information, including care team notes and test results, will appear in your account and will be visible to you and anyone with proxy access to your account. You may review your account and make changes to proxy access through the Share My Record feature in MyChart. For assistance with requesting or revoking proxy access, please contact QuadMed Patient Services at patientsupport@quadmedical.com or call 888.417.1001.
