What is MyChart?

MyChart is a secure, online patient portal that allows you to manage your health and connect with your workplace health center operated by QuadMed. MyChart has many convenient features including the ability to:

  • • Schedule, review, cancel and change appointments
  • • View your health summary, including history, medications, allergies, immunizations, provider notes, plan of care, and visit notes
  • • View test results
  • • Communicate electronically and securely with your health care provider and care team
  • • Request prescription renewals
  • • eCheck-in for most appointments
  • • Pay your bills online

How do I sign up?

To sign up for MyChart, you will need an authorization code that is generated just for you. There are several ways to obtain one:

  • 1. Visit and click "SIGN UP NOW"
  • 2. Refer to a recent patient statement or After Visit Summary (AVS) for a MyChart activation code
  • 3. Request a MyChart activation code during your next visit to the health center

The code is generated just for you to log in and create your own user ID and password. MyChart is a free service offered to our patients.

Can I view a family member's health record in MyChart?

Yes, this is called proxy access. Proxy access allows a patient to log in to their personal MyChart account, and then connect to information regarding a family member. Proxy access can be requested in the health center or through MyChart.

Can my spouse and I share one MyChart account?

No. We are committed to protecting the privacy and confidentiality of each individual's health information. For that reason, each adult must have their own MyChart account. However, members of the same family can grant proxy access to other members of their family from within MyChart.

Can I access my child’s health information in MyChart?

Yes, your child’s health care can be easily managed through your own MyChart account with convenient proxy access. Due to patient confidentiality laws in certain states, the type of proxy access is determined based on the age of the child.

Ages 0-11

Parents or legal guardians will be granted full access to the patient’s MyChart record, including scheduling and messaging.

Ages 12-17

Parents or legal guardians will be granted scheduling and messaging access to the patient’s MyChart record. Proxy access does not include access to the clinical features of the patient’s MyChart record.

Age 18 & Older

At age 18, parents or legal guardians no longer have access to their dependent’s MyChart record. Once the child reaches age 18, they must create their own MyChart user ID and password. They will then have the ability to grant proxy access to family members by sending a proxy invitation from their own MyChart account.

How can I see who has proxy access to my MyChart Account?

Log in to your QuadMed MyChart account and click on the Health icon on the top bar of your MyChart Account. Then, click Share My Record under Medical Tools. This will show you who has proxy access to your MyChart. If you wish to change (revoke) who has access to your records, you may do that here.

If I already have a MyChart account with a different health system, do I need to create a separate account for QuadMed?

Yes, you will need to create a separate MyChart account for QuadMed. Once your QuadMed account is created, MyChart will recognize if you have an account at another provider organization and will prompt you to link your MyChart accounts if you wish. Some features, such as scheduling, provider messaging and viewing test results, are organization-specific. To access these functions, you will need to log in to your MyChart account at the respective provider organization.

Can I access information from QuadMed's previous patient portal?

Many of your records will be available in MyChart. Some information including past secure messages with your provider will not transfer over to MyChart. If you're looking for medical records but don't see them in your MyChart account, you may request a copy of your records by visiting and completing the request form.

When can I see my test results in MyChart?

Most of your test results are released immediately to your MyChart account. Certain test results are not released to your MyChart account until your provider has reviewed them. This allows your provider to deliver the results with the proper personal communication and explanation. These test results may take up to seven days to be seen in your MyChart account.

If there is incorrect information in my MyChart account, what should I do?

MyChart displays information from QuadMed's electronic medical record. If you feel your information is incorrect, please contact your health center or discuss with your provider directly during your next visit.

Is information in MyChart confidential? Will it be shared with my employer?

MyChart is a secure portal that makes it easy and convenient for patients to access the health information contained in their medical record. Your MyChart account information is not accessible to your employer.

Can I access Wellness Online through MyChart?

Yes, once you log in to MyChart, click "Health" on the top menu bar, and choose "Wellness Online Portal.”
