What is MyChart?

MyChart is a secure, online patient portal that allows you to manage your health and connect with your workplace health center managed by QuadMed. MyChart has many convenient benefits including:

  • Schedule, cancel and change appointments
  • View test results
  • Securely communicate with your provider
  • Request prescription refills
  • Pre check-in for most appointments
  • View health history
  • Pay bills online

How do I sign up?

Start by visiting mychart.myquadmedical.com and clicking "Sign Up Now." If you received an activation code while visiting the health center or on your patient bill, you can enter that and a few other pieces of information, which will allow MyChart to recognize you. If you do not have an activation code, just click "Don't have an activation code? Sign up online." on the right side panel. This method will require you to enter a few more pieces of information about yourself. Once that is complete, you will need to confirm the account setup from the automated email that is sent to the email address linked to your account.

If I already have a MyChart account with a different health system, do I need to make a separate one for QuadMed?

Yes, you will need to create a separate account for QuadMed. You can link your accounts using the Happy Together feature, which allows you to see combined views of your medical records without needing to log into different portals. When you create your account with QuadMed, MyChart will recognize that you have another account and will prompt the request to link accounts.

Some features, such as scheduling, secure messaging and viewing lab results, are site-specific. To access these functions, you will need to log into the respective account. 

Can I access information from QuadMed's previous patient portal?

Many of your records will be available in MyChart. Some information including past secure messages with your provider will not transfer over to MyChart. If you're looking for medical records but don't see them in your MyChart account, you may request a copy of your records by visiting myquadmedical.com/content/medical-records and completing the request form.

When can I see my test results in MyChart?

Most test results will be released to your MyChart account 24 to 72 hours after the final results. Certain test results are not released to your MyChart account until your provider has reviewed them.

Why are certain test results not shared in MyChart?

Test results of a very sensitive nature are not released to MyChart. This allows your provider to deliver the results with the proper personal communication and explanation.

If there is incorrect information in my MyChart account, what should I do?

MyChart displays information from QuadMed's electronic medical record. If you feel your information is incorrect, please contact your health center or discuss with your provider directly during your next visit.

Can I view a family member's health record in MyChart?

Yes, this is called proxy access. Proxy status allows access to health records, test results, appointment scheduling and more on behalf of the patient. In the case of a proxy for a minor child ages 0-11, the minor’s record will be accessed through the parent or legal guardian’s MyChart account. In the case of adult-to-adult proxy access, proxy designation can be initiated by the patient from within their MyChart account. The designated proxy will receive an electronic notification from MyChart containing instructions for signing up as a proxy. Depending on the proxy's relationship to the patient, additional documentation may be required as applicable. Documentation can be mailed, emailed, or faxed to:

Can I have a MyChart account for my children?

Yes, QuadMed has established a consistent nationwide practice that allows parents or guardians to establish a proxy account for minor children between the ages of 0-11 and adult children 18 and older. Due to patient privacy and confidentiality laws varying from state to state as well as do minors' rights to access and consent to health care, MyChart accounts are not available for minor children between the ages of 12-17. Appointments and other requests for information for adolescents in this age range may still be initiated by parents or guardians in person or by phone. All patients 18 and older are encouraged to create and maintain their own MyChart account.

Can my spouse and I share one MyChart account?

No. We are committed to protecting the privacy and confidentiality of each individual's health information. For that reason, each adult must have their own MyChart account. However, members of the same family can grant proxy access to their MyChart account.

Is information in MyChart confidential? Will it be shared with my employer?

MyChart is a secure portal that makes it easy and convenient for patients to access the health information contained in their medical record. Your MyChart account information is not accessible to your employer.

Can I access Wellness Online through MyChart?

Yes, once you log into MyChart, click "Health" on the top menu bar, and choose "Wellness Online Portal".
